Allergy Treatment

Immunotherapy is a preventative treatment for allergies caused by common allergens like pollen, dust mites, molds, and animal dander. Unlike over-the-counter medications that only alleviate symptoms, immunotherapy addresses the underlying cause of allergic reactions. It involves gradually introducing increasing doses of specific allergens to help your immune system build natural immunity. This process can ultimately reduce allergic reactions without the need for ongoing medication.

Immigration Physicals and Vaccinations

An immigration medical exam is a required part of the U.S. immigration process for individuals applying for permanent residency (green card). This exam ensures public health safety and removes any medical grounds of inadmissibility for applicant.

Certain communicable diseases can make an individual inadmissible to the United States. The immigration physical exam helps verify vaccination status, screen for public health conditions, and complete the required USCIS Form I-693.

What is included in the immigration physicals?

  • Basic Physical Examination
  • Required blood work, including Tuberculosis (TB) skin testing
  • Verification of vaccination status
  • Administration of necessary vaccinations, such as:
  • Tetanus
  • Measles, Mumps, and Rubella
  • Copy of Laboratory Results
  • Certified and sealed I-693 Form

What to Bring to Your Appointment:

  • A valid photo ID (U.S. driver’s license or passport). Please bring a copy along with the original.
  • I-693 forms (available online at the USCIS website, do not sign)
    1. Please print single-sided.
    2. Type your information on pages 1 and 2 (do NOT sign).
    3. From page 3 onwards, fill in your name and A-Number (if applicable) at the top.
    4. On Page 5 please fill the Doctor's information as available on the website for speeding up the process.
  • All Immunization records, including Covid-19 vaccine
All documents must be in English. If you have vaccination records in other languages please go to Collin County health department and get it translated.

Additional Documentation:

  • To complete the I-693 immigration medical examination, the following three components are required:
    1. 1. Communicable Disease Screening (blood and urine tests)
      1. (1) Tuberculosis (TB) — Required for applicants 2 years and older
        1. Screening is performed via QuantiFERON-TB Gold blood test.
        2. Minimum 3 ml blood sample required.
        3. Results available in 3–5 business days.
        4. TB skin test is not accepted (USCIS rule effective 10/01/2018).
        5. If TB test is positive Chest X-ray required (not included in exam fee).
        6. If chest X-ray shows any active/suspicious findings → mandatory referral to County Health Department for further evaluation/treatment.
      1. (2) Syphilis Screening
        1. Required blood test for applicants 18–44 years (per 2021 CDC guidelines).
      1. (3) Gonorrhea Screening
        1. Urine test required for applicants 18–24 years (per 2021 CDC guidelines).
    2. Physical Examination
    3. Vaccination Review & Updates. The CDC requires that all immigration applicants be up to date with certain vaccinations before USCIS will approve the I-693.
      1. If you have vaccination records:
        1. Provide documented proof including vaccine name, date, and provider.
        2. No additional vaccines may be required if all criteria are met.
      2. If you do not have records:
        1. Blood tests (titers) may be done to assess if you would need to be vaccinated for certain vaccines.
USCIS Policy Reminder: All laboratory tests must be ordered by the authorized civil surgeon at Southern Medical Associates — not by your primary care physician.
